Eight clauses, written to be pasted into a leadership handbook and adapted to your context. It puts recording, weighing, and honest correction into your practice before you need them.

Adapt freely; strike what does not fit your context. Square brackets mark the decisions your leadership team makes.
Welcome. We welcome prophetic ministry as a gift of the Spirit for edification, encouragement, and comfort (1 Corinthians 14:3), and we commit to stewarding it in a proper and orderly manner (1 Corinthians 14:40).
Recording. Every prophetic word given in our gatherings, ministry times, or prayer lines is recorded in writing at the time it is given: the content as close to verbatim as possible, the giver, the recipient, the date, and the setting.
Weighing. Every recorded word is weighed by our appointed discernment team against Scripture and the character of Jesus (1 Corinthians 14:29; 1 Thessalonians 5:20-21). Weighers record their evaluation with the word.
Confirmation. Words the team has weighed positively are confirmed by [the eldership / the senior leadership] before the church treats them as words we hold together. Confirmation is a separate step from weighing, and no individual may perform both alone.
Personal words. Words concerning marriage, family, relocation, finance, or health are given in accountable settings, recorded like all others, and offered without pressure or claim of command. The recipient remains free and responsible to test.
Stewardship. Confirmed words are reviewed by the leadership at least [quarterly]. Fulfilments are recorded and testified to publicly with thanksgiving.
Correction. Words not discerned to be from God are set aside in the record with a note. We practise honest acknowledgement of missed words, pastoral care for those who gave them, and accountability for all who minister prophetically, in keeping with the Prophetic Standards Statement.
Safeguarding. The written record of prophetic ministry is part of our safeguarding practice. No prophetic ministry happens off the record.
The policy holds because named people carry named parts of it. One person may wear more than one of these, with one exception: clause 4 keeps weighing and confirming in different hands.
Giver. Brings the word, in the settings the policy names, and submits it to weighing without pressing it.
Recorder. Writes the word down at the time it is given, with giver, recipient, date, and setting.
Weigher. One of an appointed team. Tests the word against Scripture and the character of Jesus, and records the evaluation alongside it.
Confirmer. Eldership or senior leadership. Decides what the church holds together, as a step distinct from weighing.
Steward. Reviews confirmed words on a set rhythm and records fulfilments when God answers.
Some churches run the record through a notebook and a faithful administrator. Many find a shared tool carries it further.
